Pact is currently looking for a full-time Health Coach to provide support for people with health challenges, based at the Balclutha General Practice.

Pact is a major provider of community-based mental health services across Southland, Otago, West Coast and the Lower North Island. In collaboration with WellSouth PHO, we are implementing an exciting new initiative providing additional support in General Practices with the aim of improving health outcomes.

The primary purpose of the Health Coach role is to work actively in partnership with a Health Improvement Practitioner (Registered Clinician) and the rest of the General Practice team to assist patients with health challenges to improve their general wellbeing.

This role has a service development component and is an exciting opportunity to be part of a positive new initiative in primary health.

The successful candidate will help patients to gain the knowledge, skills, tools and confidence to become active participants in their own care so that they can reach their self-identified goals. This will include but is not limited to assisting people to gain social, health, education, whanau and cultural reconnection, motivating them to make positive lifestyle changes, managing stress and reducing high risk behaviours. It will involve working with patients individually in their GP clinic as well as training, mentoring and delivering evidence based-group programmes.

We are looking for an enthusiastic and innovative person who will act as a positive role model and have the ability to motivate others.

You will have:

  • Previous experience or knowledge of the Primary Health Care or Social Services sectors.
  • A New Zealand Certificate in Health and Wellbeing or other relevant qualification is preferable but not essential.
  • Recognised Health Coach training or a commitment to undertake this training.
  • Excellent communication and interpersonal skills.
  • The ability to relate to and engage with a diverse range of people from different cultural backgrounds
  • The ability to work and interact with a wide range of stakeholders in primary care and practice environments.
  • A current full New Zealand Driver licence is an essential requirement.

We expect this position to start on the 30th of October so don’t delay your application. Suitable applicants may be interviewed prior to the closing date.
